GUIDING AN IMPROVED DEMENTIA EXPERIENCE

GUIDE is a free support program designed to make living with dementia, and providing care, less overwhelming. Through personalized guidance, ongoing check-ins, and connections to local services, GUIDE helps individuals and caregivers better navigate care and feel supported along the way.

Support for Caregivers

GUIDE provides dedicated support for caregivers, reducing stress while offering a safe space to talk through challenges. Caregivers receive encouragement, practical guidance, and resources tailored to their situation—so they feel supported and don’t have to manage dementia care alone.

Care Navigation

GUIDE offers ongoing, one-on-one care navigation to help individuals and caregivers understand options and make informed decisions. With a trusted person to call, families can ask questions, coordinate services, and receive consistent support—bringing more clarity and confidence as needs change over time.

Respite Services

When eligible, GUIDE helps connect caregivers to short-term respite services, such as in-home care or adult day programs. These options provide time to rest and recharge while ensuring loved ones continue to receive quality care, with added connections to helpful local resources and community support.

Eligibility

A person may be eligible if they have a documented diagnosis of dementia from a healthcare provider. The program includes many types of dementia, such as Alzheimer’s disease, vascular dementia, Lewy body dementia, and other related conditions, at any stage, from mild to severe. 


Eligibility also requires enrollment in Original Medicare (Parts A & B). Individuals enrolled in a Medicare Advantage plan, receiving hospice care, or living in a skilled nursing or long-term care facility are not eligible to participate.


If you or your care partner think dementia may be present but are still waiting for a specialist appointment or formal diagnosis, we encourage you to reach out. Telehealth options may be available for those who meet additional criteria and can help connect you with a specialty care team for evaluation, guidance, and support.

Scrubbing

The program is offered at no cost to eligible participants, ensuring access to needed support services.

Family

Services are covered through Original Medicare (Parts A & B), helping families avoid additional financial strain.

Going or a Walk

GUIDE staff can help determine eligibility and explain participation, making it easier for families to get started.
