Living Well with Diabetes

Practical Skills. Real Support. Better Living.

Living Well with Diabetes is a free, evidence‑based Self-Management program designed for adults living with prediabetes or type 2 diabetes. Through a supportive series of six sessions, participants gain practical, easy‑to‑understand tools to better manage blood sugar, build healthy routines, and feel more confident day to day. ​ Sessions are interactive and focus on real‑life strategies - how food, movement, stress, illness, and daily choices affect blood sugar, and what you can do to prevent or delay diabetes‑related complications. Programs are offered in person, virtually, or by phone, making it easy to participate in a way that works for you. This program is open to individuals with prediabetes or type 2 diabetes, as well as family members and caregivers who want to better support a loved one. Many participants report improved confidence, healthier habits, and better overall health outcomes, including improved A1c levels. Living Well with Diabetes is is designed to complement, not replace, care you may already receive from healthcare providers.
